Abstract

Aim.The purpose of this study was to examine the effect of patient education on postoperative pain perception in patient undergoing Coronary Artery Bypass Graft (CABG).

Background.Pain is a common complication after CABG surgery. Non-pharmacological methods are more favorable than pharmacological agents. 

Method.In this quasi-experimental study, 72 patients who were scheduled for elective CABG surgery  were randomized to experimental (n=36) and control group (n=36). Experimental group received educational intervention about pain control, one day before surgery (20-30 minutes duration), while control group received no intervention. Following transferring to the ICU and 2 hours after extubation, patients’ pain intensity wasmeasured by Johnson’s numerical scale. The measurements wererepeated twice withanintervalof threehours. The type and dosage of pain medication administered during ICU stay were recorded. To analyse the data, descriptive(mean and standard deviation, frequency distribution and percentage) and analytical statistics (Chi-square and Mann-Whitneytest) were used.

Findings.There was no statistically significant difference between two groups in terms of demographic characteristics. Two hours after extubation, there were no statistically significant difference between experimental and control group in the intensity of pain (P=0.313). Pain intensity was significantly lower in experimental group than control group, 5 hours (P=0.015) and 8 hours (P=0.006) after surgery. The results also showedthat the amount ofanalgesics used to relieve post-operativepain was significantly lower in experimental group than control group (P=0.046).

Conclusion.Patient education about pain control may have positive effects on postoperative pain in patients undergoing CABG surgery. This intervention could serve as an effective strategy for nurses to improve pain management among these patients.

Abstract

Aim. This study aimed at investigating the effect of education based on illness perception on self-care behaviors of patients with heart failure.

Background. Heart failure is the final outcome of most cardiovascular diseases, reducing patients’ self-care behaviors as a chronic condition. Changing illness perception of patients with heart failure about the disease is a way to improve the self-care behaviors in these patients.

Method. An experimental design was used to conduct the study. Seventy and six patients with heart failure who admitted to hospitals affiliated to Tehran University of Medical Sciences and were eligible to participate in the study was recruited in 2014 and were randomly divided into intervention and control group. The intervention group attended in three educational sessions and received cell phone follow-up for 8 weeks. The control group received conventional treatment. The self-care behaviors was compared in both groups before, and eight weeks after the intervention. The data were analyzed by SPSS-16 software.

Findings. There was no statistical significant difference between demographic variables of two groups. Self-care behaviors in the intervention group increased after the intervention when compared with the control group (p<0.0001). In the experimental group, the mean score of self-care behaviors after the intervention was significantly higher than mean score before intervention (p<0.0001).

Conclusion. The results showed that education based on illness perception affects self-care behaviors in patients with heart failure. It is suggested as an effective method to promote the self-care behaviors in these patients.

Abstract

Abstract
Aim. This study aimed to evaluate the perceptions of nurses working in intensive care units about obstacles and supportive behaviors of end of life care.
Background. End of life care is an important part of nursing duties in intensive care units. Exploring supportive behaviors of end of life care from the perspective of nurses may lead to better understanding of supportive behaviors and obstacles of end of life care.
Method. This study was a descriptive cross-sectional study in which 160 staff nurses, working in intensive care units in Rajaei Cardiovascular Medical and Research Center, were recruited by random sampling in 2016. Data were collected using demographic form and the questionnaire “National Survey of Critical-Care Nurses Regarding End-of-Life Care” (NSCCNR-EOL) and analyzed in SPSS version 22 software using descriptive statistics.
Findings. The most important obstacle in end-of-life care was "family and friends who continually call the nurse wanting an update on the patient's". In the field of supportive care, "After the patient's death, having support staff compile all the necessary paper work for you" received the highest score.
Conclusion. Based on the findings, in critical care nurses views, appropriate training on end-of-life care and palliative care for staff, educating families about issues of death and dying, and the holistic care of patients and their families improve end-of-life care of dying patients.

Abstract

Abstract
Aim. The aim of this study was to examine the effect of teach back learning strategy on illness perception, nutritional knowledge and dietary adherence in people with hypertension.
Background. Because of the chronicity of hypertension and the importance of nutrition in hypertension control, illness perception and nutritional knowledge play an important role in adherence to nutritional regimen and disease control.
Method. The present study is a quasi-experimental study control group and pretest-posttest design. Based on random sampling, two health care centers in Ferdows were selected as the place for experimental and control group. Using a simple randomized method, of patients with hypertension, 35 individuals were recruited for each group. These people entered the study based on inclusion criteria and after that they signed the consent form. Before and after the intervention, illness perception, nutritional knowledge and dietary adherence were measured by self-report questionnaire and compared between the groups.
Findings. After intervention, the mean score of nutritional knowledge in experimental and control group was 48.17±5.21 and 41.16±0.51, respectively, and a statically significant difference was found between groups in terms of nutritional knowledge (P≤0.0001; t=9.56). After intervention, the mean score of illness perception in experimental and control group was 9.75±1.81 and 7.72±1.60, respectively, and there was a statically significant difference between groups in terms of illness perception (P≤0.0001; t=4.15). But no statically significant difference was found between experimental and control group in terms of dietary adherence (17.55±4.17 against 16.88±3.62).
Conclusion. Given the positive effects of teach back method and the special role of nurses in patient education, attention to this teaching method seems necessary. The use of this method of training is suggested as part of care process to reduce patients' clinical problems.

Abstract

Abstract
Aim. The present study was conducted with the aim of examining the effect of multimedia education with the Leventhal model approach based on follow-up on understanding of patients with atrial fibrillation.
Background. One of the basic tools in changing the patient's perception of the disease is the existence of an educational program using modern teaching methods.
Methods. In this semi-experimental study, 48 patients with atrial fibrillation were randomly divided into intervention (24 people) and control (24 people) group in Kashani Shahrekord Hospital in 2022. In the intervention group, two virtual training sessions were held for patients through multimedia software (sound, video, image and animation) under the title of 4-hour training workshop on the date and time coordinated with the help of Adobe Connect program. There was no intervention in the control group. After the workshop, disease perception were measured using disease perception questionnaire. Data analysis was done using independent t and paired t tests in SPSS version 21.
Findings. There was no statistically significant difference in the mean score of disease perception between the two intervention and control group before the study, and the two groups were homogeneous. However, after implementation of the intervention, the mean score of disease perception was significantly difference between the groups (p≤0.0001).
Conclusion. Based on the results of the present study, patients with atrial fibrillation gained a better understanding of their disease after the educational intervention. Therefore, it is recommended to carry out educational interventions to improve the understanding of patients with atrial fibrillation.
