Aim. The aim of this study was to determine the current quality of triage in intensive care units for patients admitted to hospital emergency departments in ShahidBeheshti University of Medical Sciences in 2012.

Background. Accurate and rapid patient triage in emergency departments is the key to a successful performance. The study and understanding of the current condition and evaluation  of the  educational needs of triage nurses are highly important.

Method. This study is a retrospective descriptive study in which the data were gathered via in a list of ESI  triage forms. To determine the content validity of data collection tools was used. In this study, the reliability of the test listed under triage nurse was both between observers and reliability with Intraclass Correlation Coefficient (ICC) : 0.857 was specified.

Findings. In this study,23% of the case were undertriage and 11.7% were overtriage.The most of undertriage cases was level 3 based on Emergency Severity Index  (ESI) system. There is a direct correlation between the mortality and hospitalization in the Intensive Care Unit (ICU) and undertriage.

Conclusion. There was no statistically significant difference between proper triage and undertriage cases in  mean length of hospital stay, duration of hospitalization and emergency department waiting time for the first visit of  the emergency physician.

Abstract

Aim. The purpose of this paper is to describe different types of primary triage methods that are common and applicable in disaster scene in order to familiarize medical team personnel with these methods.

Background. Disaster occurrence is rising all over the world. Primary assessment, treatment and transfer of victims to the medical centers which are away from the disaster scene is important considerations for first responder teams in response phase of disaster management cycle. In this regard, triage is an efficient tool for mass casualty management for which different strategies have been recommended in various countries.

Method. In this review, published articles from 1995 to August 2013 were searched in Ovid, Science Direct, Google Scholar and PubMed data bases by using the keywords Triage, On-scene and Disaster.

Findings. Based on search strategy, 50 English articles were found. Finally, 6 commonly adults triage methods and 2 pediatric triage methods were chosen to be described in this paper.

Conclusion. Regarding the sudden nature of disaster and creating chaos following them, it is necessary for medical team members to apply the appropriate tools for managing mass casualties in attention to limited resources and the large number of victims. In order to perform proper triage method and reduce the amount of error in this procedure, it is necessary to educate correct practices of applicable triage methods to medical team members, Red Crescent workers and volunteers.

Aim. This review study aims to investigate the impact of reducing Door-to-ECG (DTE) time on on morbidity and mortality of patients with chest pain referred to emergency department.
Background. Chest pain is the second most common reason for referral to an emergency unit. According to American Heart Association, all patients with chest pain presenting to the emergency ward should be approached with an ECG within 10 minutes of entrance to the emergency department. This is considered as the first line of diagnostic tests for myocardial infarction.
Method. In this narrative review study, several databases and search engines including SID, Pub Med, Google Scholar, Web of Science and Scopus were used to find the articles published between 2000 and 2018. The keywords used to search articles were “Door to ECG”, “chest pain”, “emergency unit” and “triage”. The articles selected for review were interventional or descriptive in terms of design, in Persian or English, with available full text. The criterion for selecting articles was the similarity of the their subject with the search keywords including Door to ECG (DTE) time and triage and their value in the early diagnosis and treatment of coronary disorders.
Findings. A total of 20 articles were found. After removing the articles that didn’t meet the inclusion criteria, 11 articles related to the subject of the study remained in review. The significant outcomes of timely triage, appropriate time to perform ECG, barriers to access and suggestions for faster treatment by the system especially the role of nurses, were investigated.
Conclusion. The reduction of triage time and Door to ECG (DTE) time as well as the role of the nurse in those cases are associated with a reduction in the morbidity and mortality of cardiac patients.
